CHC is a leading nonprofit healthcare provider based in Middletown, Connecticut. As a Federally Qualified Health Center, CHC delivers comprehensive, high-quality care to medically underserved communities across Connecticut, within both traditional clinics and a variety of community settings such as schools, shelters, mobile units, and more. Founded in 1972, CHC now serves over 145,000 patients using an integrated model that combines medical, dental, and behavioral health services, with additional specialty programs including nutrition, women's health, and chronic disease management. CHC is part of the Moses/Weitzman Health System, the first health system in the United States focused on primary care.
